About the Role
As a Support Coordinator, you will work closely with NDIS participants to ensure their individual needs are met through tailored support and service coordination. You’ll advocate on behalf of clients, manage resources effectively, and maintain strong communication with all stakeholders to deliver person-centred outcomes.
Key Responsibilities
Client Advocacy – Champion the goals and preferences of our clients, ensuring all services reflect their individual aspirations.Service Coordination – Coordinate health, housing, and social services in alignment with each participant’s care plan.Resource Management – Use NDIS funding wisely and efficiently to maximise positive outcomes.Documentation & Reporting – Keep detailed and accurate records and prepare reports to meet compliance and track impact.Stakeholder Engagement – Build strong, ongoing relationships with participants, families, providers, and other stakeholders. What You’ll Bring
1–3 years of experience as a Support CoordinatorStrong knowledge of the NDIS framework, guidelines, and policiesA confident and empathetic approach to advocacyExcellent interpersonal and communication skillsStrong problem-solving, time management, and organisational abilitiesCommitment to client-centred care and outcomesPatience and compassion when working with individuals with diverse and complex needsQualifications & ChecksCertificate III in Individual Support, Certificate IV in Disability, or Diploma of Community ServicesNDIS Clearance Check and NSW Working with Children Check (or willingness to obtain)Current and valid Australian driver’s licenceValid Australian working rights
